Food Safety Self Inspection Forms and Logs


These forms are in two different formats, Adobe Acrobat and Microsoft Excel. The Excel files can be modified to meet your establishment's particular needs.

The Food Safety Self Inspection Checklist can be used to evaluate food safety problems in an establishment. It can also be used as an educational tool with employees as each area is checked.

Adobe Acrobat
The Food Safety Self Inspection Reference gives the Kansas Food Code reference number and whether the item is a critical violation. Health Inspectors typically focus on Critical Violations as they are more likely to cause foodborne illness.
